Job summary
Wheeler Health in New Haven, CT is seeking a Diversion First coordinator to support youth diverted from juvenile court through referral screening, case management and collaboration with community services.

Qualifications
- BA/BS degree in social work, criminal justice, psychology, counseling or related field required.
- Must have a valid driver's license and access to reliable transportation.
Responsibilities
- Provides coordination for referrals and maintains linkage with community sources.
- Develops relationships with Police, Schools, Juvenile Probation, DCF and area service providers.
- Provides direct case management and develops plans to increase treatment completion and life functioning.
- Maintains case notes, forms and documentation for medical records and reporting.
- Determines eligibility for the YDT and secures parental/youth agreement to participate.
- Performs screenings and prepares summary reports for team review.
- Occasionally transports clients and provides translation support as needed.
- Participates in team and staff meetings to coordinate case management.
