Job summary

The Village for Families and Children in Hartford, CT is seeking a Mid-Career Case Manager to support the Youth Diversion Team. You will provide comprehensive case management, connect clients to community services, and collaborate with schools, police, and social services to meet referral requirements.

Requires a BA/BS and 2–5 years of experience in behavioral health or substance use treatment; bilingual English/Spanish preferred.

Qualifications

Bachelor's degree required.
At least 3 years of direct experience in SUD treatment or related behavioral health fields.
Culturally competent to serve diverse client populations; bilingual (English/Spanish) strongly preferred.
Excellent oral and written communication skills.
Organized and able to work independently with minimal supervision.

Responsibilities

Provide comprehensive case management services to clients and families.
Assist clients in gaining eligibility for entitlement programs and connecting to services.
Maintain and update documentation in compliance with HIPAA and funding sources.
Collaborate with police, schools, probation, DCF, and community providers.
Develop and maintain collaborative relationships with community agencies.
Ensure proper reports and screenings are completed for Youth Diversion Team programs.
