Responsibilities

• Engage clients from referral through completion of treatment services.
• Assist clients in connecting with entitlement programs and insurance.
• Conduct standardized health-related social needs screenings and referrals.
• Provide linkage and navigation to community services for clients and families.
• Maintain and update service data for accurate reporting.
• Develop collaborative relationships with community agencies and referral sources.
• Ensure compliance with agency policies, HIPAA, and Joint Commission standards.
