Job summary

The Village for Families and Children is seeking a Case Manager in Hartford, CT to deliver comprehensive case management for clients and families within the Youth Diversion Team. You will collaborate with law enforcement, schools, and service providers to ensure access to community-based services and compliance with referrals.

Required: BA/BS, 3+ years in SUD treatment, bilingual English/Spanish preferred, and strong written/oral communication.

Qualifications

• Bachelor’s degree required.
• At least 3 years of direct experience in SUD treatment with familiarity with CM protocols and evidence-based practices.
• Must be culturally competent to serve diverse client population; bi-lingual (English/Spanish) strongly preferred.
• Excellent oral and written skills and ability to communicate with children, parents, and professionals.

Responsibilities

• Focuses on engaging clients from referral through treatment services.
• Assists clients in connecting with insurance programs and entitlement services.
• Conducts standardized screening for health-related social needs and offers resources.
• Provides linkage and navigation for clients to access community services.
• Helps clients and families access Village clinical services and community resources.
• Maintains service data for accurate reporting when required.
• Develops collaborative relationships with community agencies and referral sources.
• Complies with HIPAA, Joint Commission standards, and funding requirements; documents thoroughly.
• Demonstrates cultural competence to serve diverse populations.
• Participates in team-based quality improvement efforts.
• Represents The Village in community groups to enhance visibility and linkages.
