Job summary

ROWI Teen & Parent Wellness Centers is seeking a Care Coordinator to manage the coordination of care for adolescents and families. Under the supervision of the Program Director, you will arrange orientation, transportation, doctor visits, and scheduling to ensure seamless services.

You will support compliance with standards, document in EMR, and coordinate client transportation while maintaining professional boundaries and timely reporting.

Qualifications

• Minimum high school diploma; Bachelor’s degree preferred.
• Minimum of two years driving experience required.
• Minimum of 1 year working with kids in a therapeutic setting.
• Experience with junior high or high school students preferred.
• Must communicate with people from diverse backgrounds.
• Strong written and verbal communication skills.

Responsibilities

• Coordinate individualized services including orientation, transportation, doctor visits and scheduling.
• Ensure programs and services meet high standards and clients receive quality care.
• Supervise clients and ensure attendance at designated groups.
• Facilitate groups and activities as directed by the Facility Manager.
• Conduct client orientation and assessments on admission day.
