Job summary

ROWI Teen & Parent Wellness Centers is seeking a dedicated Care Coordinator to oversee care coordination for all ROWI clients, including orientation, transportation, doctor visits, and scheduling. You will work under the Program Director to ensure high standards of care and coordinate individualized services.

Responsibilities include client supervision, group facilitation, attendance, and accurate documentation in EMR.

Qualifications

• Requires ability to manage time effectively and work with minimal supervision.
• Strong written and verbal communication with families and professionals.
• Proficient in basic computer skills and EMR documentation.
• Demonstrates tact, initiative, and a professional attitude.

Responsibilities

• Coordinate care for ROWI clients including orientation, transportation, doctor visits, and scheduling.
• Ensure programs and services meet high standards and clients receive quality care.
• Supervise clients and ensure attendance in designated groups.
• Conduct client orientation, including assessments as directed.
• Maintain EMR documentation with timely entries (within 24 hours).
• Coordinate transportation and drive clients as needed; document in EMR.
