ROWI Teen & Parent Wellness Centers seeks a dedicated Care Coordinator to manage care coordination for ROWI clients. You will organize orientations, transportation, doctor visits, and scheduling while upholding standards of care.

The role requires strong communication, group facilitation, and ability to work within an interdisciplinary team. A high school diploma is required, with a preference for a Bachelor’s degree and at least 1–2 years in a therapeutic setting.

Formación

• Minimum high school diploma; bachelor’s degree preferred.
• Minimum 2 years driving experience.
• Minimum 1 year working with kids in a therapeutic setting.

Responsabilidades

• Coordinate individualized services including orientation, transportation, and doctor visits.
• Ensure clients attend designated groups and activities.
• Conduct client orientation on admission and perform assessments as directed.
• Maintain EMR documentation and submit reports within 24 hours.
• Coordinate transportation schedules and drive clients as needed.
• Uphold Joint Commission standards and facility cleanliness.
