Job summary

ROWI Teen & Parent Wellness Centers is seeking a Care Coordinator to ensure the highest standard of care for clients. Under the supervision of the Program Director you will coordinate orientation, transportation, doctor visits, and scheduling, while maintaining confidentiality and professional boundaries.

You will lead group facilitation, document in EMR, and ensure shifts are reported promptly. The role requires at least a high school diploma (Bachelor’s preferred), two years driving

Qualifications

• Minimum two years driving experience.
• Minimum of 1 year working with kids in a therapeutic setting.
• Experience with junior high or high school students preferred.
• Able to communicate with people from diverse backgrounds.
• Must cope constructively with stressful situations.
• Strong written and verbal communication.
• Documents clearly and efficiently.
• Strong interpersonal and organizational skills.

Responsibilities

• Coordinate individualized services including orientation and transportation.
• Ensure clients attend designated groups.
• Facilitate groups and activities per Facility Manager.
• Conduct client orientation on admission day.
• Ensure compliance with Joint Commission Standards.
• Document shift reports in EMR.
• Coordinate transportation schedule and drive clients as needed.
• Submit documentation in EMR within 24 hours.
• Assist with facility cleanliness.
• Maintain confidentiality of clients.
