Job summary

ROWI Teen & Parent Wellness Centers is seeking a Care Coordinator to manage care coordination for clients, including orientation, transportation, doctor visits, and scheduling. You will ensure clients attend groups and participate in programming while maintaining confidentiality and documenting activities in EMR.

Under supervision of the Program Director, you will drive when needed, coordinate transportation, and collaborate with a multidisciplinary team to deliver high-quality, compassionate

Qualifications

• Minimum of two years driving experience.
• Minimum of 1 year working with kids in a therapeutic setting.
• Experience with junior high or high school students is preferred.
• Must be able to communicate with people from all educational and cultural backgrounds.

Responsibilities

• Coordinate care for ROWI clients including orientation, transportation, doctor visits, and scheduling.
• Ensure clients attend designated groups and participate in programming.
• Document in EMR; prepare shift reports; maintain confidentiality.
• Drive clients as needed and manage transportation schedules.
• Maintain professional attitude and collaborate with staff.
