ROWI Teen & Parent Wellness Centers seeks a Care Coordinator to manage care coordination for adolescents, including orientation, transportation, and medical visits. The role reports to the Program Director and emphasizes compassionate, professional client support.

The position requires strong written and verbal communication, ability to work in a team, and experience with therapeutic settings. On-site role with standard business hours and a focus on patient well-being.

Qualifications

• Requires at least a high school diploma; Bachelor's preferred.
• Minimum of 2 years driving experience.
• Minimum 1 year working with kids in a therapeutic setting.
• Experience with junior high or high school students preferred.
• Ability to communicate with people from diverse backgrounds.
• Must cope well with stressful situations.
• Demonstrates tact, initiative, and professionalism.
• Strong written communication and organizational skills.

Responsabilités

• Coordinate care for ROWI clients; oversee orientation, transportation, doctor visits, and schedule.
• Ensure services are delivered to high standards and clients receive quality care.
• Supervise clients and ensure attendance at designated groups.
• Facilitate groups and activities as directed by Facility Manager.
• Document shift reports and share information with the team.
• Coordinate transportation and drive clients as needed; document in EMR.
