Job summary

ROWI Teen & Parent Wellness Centers is seeking a Care Coordinator to manage care coordination for adolescents. You will orient clients, arrange transportation, and coordinate doctor visits within a supportive, team-oriented setting.

The role emphasizes maintaining boundaries, documenting in EMR, and ensuring safety and compliance while working with families and school-age students.

Qualifications

• High school diploma or higher; bachelor’s degree preferred.
• Minimum of two years driving experience.
• At least 1 year working with kids in a therapeutic setting.
• Experience with junior high or high school students preferred.
• Ability to communicate with people from diverse backgrounds.
• Strong written communication and professional demeanor.
• Ability to manage stress and maintain boundaries.

Responsibilities

• Coordinate care for ROWI clients including orientation and transportation.
• Ensure programs and services are implemented at high standards.
• Facilitate groups and activities as designated by the Facility Manager.
• Conduct client orientation and complete assessments as directed.
• Maintain EMR documentation and client confidentiality.
• Coordinate transportation and drive clients as needed.
• Assist with facility cleanliness and coordinate client lunch.
• Attend staff meetings and perform additional duties.
• Maintain professional boundaries with clients and staff.
• Submit all EMR documentation within 24 hours.
