Job summary

ROWI Teen & Parent Wellness Centers seeks a Care Coordinator to manage the coordination of care for ROWI clients, including orientation, transportation, doctor visits, and scheduling. The role supports a culture of care and requires maintaining high standards in a collaborative setting.

The position emphasizes client supervision, meeting Joint Commission standards, and timely documentation in EMR, with a focus on safety and compassionate service for adolescents and families.

Qualifications

• High school diploma required, Bachelor’s preferred.
• Two years driving experience required.
• At least 1 year working with kids in a therapeutic setting.
• Experience with junior high or high school students preferred.

Responsibilities

• Coordinate individualized services including orientation, transportation, doctor visits and scheduling.
• Ensure programs and services meet high care standards and client needs.
• Supervise clients and ensure attendance at designated groups.
• Conduct client orientation and assessments (e.g., UA screening, PHQ9).
• Coordinate transportation and document activities in EMR.
• Maintain confidentiality and professional conduct; follow Joint Commission Standards.
