Job summary

ROWI Teen & Parent Wellness Centers is seeking a Care Coordinator to manage coordination of care for clients, including orientation, transportation, and medical visits. The role requires strong organizational and communication skills within an interdisciplinary team.

Responsibilities include supervising clients, facilitating groups, completing assessments, and ensuring documentation compliance within 24 hours in the EMR.

Qualifications

• Minimum of two years driving experience.
• Minimum of 1 year working with kids in a therapeutic setting.
• Experience with junior high or high school students preferred.
• Must communicate with people from diverse backgrounds.
• Must have a valid driver’s license and eligibility for agency insurance.
• First-Aid & CPR certification or willingness to obtain.

Responsibilities

• Coordinate individualized service including orientation, transportation, and doctor visits.
• Supervise clients and ensure attendance at designated groups.
• Facilitate groups and activities as directed by the Facility Manager.
• Conduct client orientation and required assessments.
• Ensure compliance with Joint Commission Standards.
• Document shift reports and share pertinent information with the team.
• Coordinate transportation schedule and drive clients as needed.
• Document transportation and group activity in EMR.
• Document EMR for client Urine Analysis.
• Maintain cleanliness and coordinate client lunch daily.
• Serve as a professional role model and maintain confidentiality.
• Attend staff and in-service meetings.
• Complete additional duties as assigned by management.
• Maintain clear boundaries with clients and staff; ensure safe supervision.
