Job summary

ROWI Teen & Parent Wellness Centers is seeking a Care Coordinator to manage coordination of care for all ROWI clients. The role includes orientation, transportation, doctor visits, and scheduling, with supervision of clients and group activities.

You will conduct client orientation, complete UA screening and PHQ9 as directed, maintain EMR documentation, and ensure Joint Commission standards compliance. A strong, professional demeanor is essential in a youth-focused setting.

Qualifications

• Minimum a high school diploma; bachelor’s degree preferred.
• Minimum of 2 years driving experience.
• Minimum of 1 year working with kids in a therapeutic setting.
• Experience with junior high or high school students is preferred.

Responsibilities

• Coordinate individualized care including orientation, transportation, doctor visits, and scheduling.
• supervise clients and ensure attendance at designated groups.
• Facilitate groups and activities as designated by the Facility Manager.
• Conduct client orientation on admission; perform UA screening and PHQ9 as directed.
• Ensure compliance with Joint Commission Standards.
• Document shift reports and EMR entries; share pertinent information with the team.
• Coordinate transportation schedule for all clients and drive clients as needed.
• Document all transportation and group activity in EMR.
• Maintain confidentiality and professional boundaries; attend staff meetings.
• Attend designated staff and in-service meetings; perform other duties as assigned.
