Job summary

ROWI Teen & Parent Wellness Centers is seeking a Care Coordinator to oversee coordination of care for all ROWI clients. You will arrange orientation, transportation, doctor visits, and scheduling under the supervision of the Program Director.

Responsibilities include supervising clients in groups, facilitating activities, conducting admissions orientation, and ensuring EMR documentation is completed within 24 hours. A positive, professional attitude is essential.

Qualifications

• High school diploma required; Bachelor’s preferred.
• Minimum 2 years driving experience.
• Minimum 1 year working with kids in a therapeutic setting.
• Experience with junior high or high school students preferred.
• Ability to communicate with people from diverse backgrounds.

Responsibilities

• Coordinate care for ROWI clients, including orientation and transportation.
• Supervise clients and ensure attendance at designated groups.
• Facilitate groups and activities as assigned by the Facility Manager.
• Conduct client orientation on admission day and perform assessments as directed.
• Ensure compliance with standards and document shift details.
• Coordinate transportation schedules and drive clients as needed.
• Document transportation and group activity in EMR within 24 hours.
• Assist with facility cleanliness and prepare daily client meals.
• Serve as a role model through a caring, professional attitude.
