ROWI Teen & Parent Wellness Centers is seeking a Care Coordinator to manage coordination of care for all ROWI clients, including orientation, transportation, doctor visits, and scheduling. You will collaborate with the Program Director and treatment staff to ensure a high standard of care and smooth group activities in a supportive environment.

The role requires experience with kids in therapeutic settings, strong written and verbal communication, and the ability to work with diverse families

Formación

• Minimum of a high school diploma; Bachelor’s degree preferred.
• Minimum of two (2) years driving experience.
• Minimum of 1-year working with kids in a therapeutic setting.
• Experience with junior high or high school students preferred.
• Must be able to communicate with people from diverse backgrounds.
• Must cope constructively with stressful situations.
• Strong written communication skills.
• Excellent verbal communication skills.

Responsabilidades

• Coordinate care for ROWI clients under the supervision of the Program Director.
• Coordinate orientation, transportation, doctor visits, and scheduling.
• Ensure programs and services meet high standards and clients receive quality care.
• Supervise clients and ensure attendance at designated groups.
• Conduct client orientation on admission day and perform assessments as directed.
• Maintain compliance with Joint Commission Standards and document shift reports.
• Coordinate transportation and drive clients as needed; document in EMR.
• Document all transportation and group activity in EMR; handle Urine Analysis documentation.
• Assist with facility cleanliness and coordinate daily client lunch.
• Be a role model with a positive, caring, professional attitude; maintain client confidentiality.
