Job summary

ROWI Teen & Parent Wellness Centers is seeking a Care Coordinator to support the coordination of care for all ROWI clients. Under the supervision of the Program Director, you will organize orientation, transportation, doctor visits, and scheduling to ensure smooth service delivery.

You will supervise clients, facilitate groups as directed, document in EMR, and maintain professional boundaries while working with a multidisciplinary team to provide high-quality care within a therapeutic setting.

Qualifications

• Minimum of a high school diploma; Bachelor’s degree preferred.
• Minimum of two years driving experience.
• Minimum of 1 year working with kids in a therapeutic setting.

Responsibilities

• Coordinate individualized service including orientation, transportation, doctor visits, and scheduling.
• Ensure programs and services are implemented to high standards and clients receive quality care.
• Supervise clients and ensure attendance at designated groups throughout programming.
• Document shift reports and share pertinent information with the team.
• Coordinate transportation schedules and drive clients as needed.
• Maintain EMR documentation for client Urine Analysis and daily activities.
