Job summary

Elite Care Management in Yorkville, Illinois is seeking an RN or LPN for part-time/PRN home health care. The role offers a $6,000 sign-on bonus for FT and $3,000 for PT, with a pay range of $35–$40 per hour and shifts 7p–7a on Saturdays on a rotation.

You will provide one-on-one care in patients' homes, with comprehensive benefits, training, and ongoing clinical support.

Qualifications

• Valid driver’s license is required.
• Current CPR certification is required.
• TB test: 2-step or chest x-ray within past 12 months or QuantiFERON Gold in last 5 years.

Responsibilities

• Observe patients and report significant conditions or changes to the supervisor or physician.
• Follow nursing policies and procedures precisely.
• Communicate clearly, both verbally and in writing.
• Deliver patient care per the established care plan and update as necessary.
