Job summary

Health Connect America is seeking a Wraparound Facilitator to deliver best-practice, trauma-informed community support services and ensure documentation meets payer, state, and COA standards. The role includes marketing new referrals and coordinating with treatment teams.

You will connect families with community resources, document progress, and maintain active caseloads while meeting weekly or bi-weekly with clients for up to 12 months, and participating in team meetings.

Qualifications

• Communicates with customers, colleagues and other individuals to answer questions and address concerns.
• Develops service plans with input from the treatment team and the Clinical Supervisor.
• Connects families with community resources and follows up on referrals.

Responsibilities

• Provide best practice, quality community support services and ensure documentation per payer, state and COA standards.
• Communicate with customers, colleagues, and other individuals to answer questions, disseminate or explain information, and address complaints/concerns.
• Develop service plans with input of the treatment team and the Clinical Supervisor
• Connect families with needed and available community resources. Follow-up with clients and agencies as appropriate to document use/success of referral.
• Complete documentation within established time frames
• Meet with clients weekly or bi-weekly for up to 12 months.
• Attend and participate bi-weekly with Treatment Team meetings
• Serve as a liaison with other professionals and agencies as related to the client
• Actively recruit new referrals to ensure that your caseload stays full
• Provide 24/7 on-call support for clients as per program requirements
