Job summary

Health Connect America is seeking a Wraparound Facilitator to deliver high-quality, trauma-informed community support services to families and children. You will ensure documentation aligns with payer and state standards, while coordinating treatment plans with the clinical team.

The position involves building service plans, connecting families to resources, and maintaining client caseloads with regular visits and team participation.

Qualifications

• Strong communication and collaboration abilities to coordinate with families, treatment teams and community resources.
• Ability to document and follow through on service plans with timeliness and accuracy.

Responsibilities

• Provide best-practice Wraparound services and ensure documentation meets payer, state, and COA standards.
• Develop service plans with input from the treatment team and Clinical Supervisor.
• Connect families with community resources and follow up to document use and success of referrals.
• Meet clients weekly or bi-weekly for up to 12 months; participate in bi-weekly treatment team meetings.
• Recruit new referrals to maintain caseload and provide 24/7 on-call support as required.
