Wisconsin Community Services, Inc. is seeking a Care Coordinator (Wraparound) in Milwaukee to provide intensive, strength-based care coordination for children and adolescents within the Wraparound Practice Model.

You will manage a caseload of about 8 families, coordinate team meetings, and develop initial care plans, emphasizing trauma-informed, culturally intelligent care. The role involves court appearances, resource liaison work, and collaboration with schools, families, and community

Competenze

• Must possess a bachelor's degree in a relevant area of education or human services and a minimum of one year of continued experience providing mental health services.
• Complete 85 hours of mandatory training in Wraparound philosophy and policies, as well as attend a Family Orientation within 6 months of their hire date.
• Working knowledge of computer applications preferred (Windows and Microsoft Word).
• Ability to display cultural competence by responding respectfully and effectively to people of all cultures, languages, classes, races, ethnic backgrounds, religions, and other diversity factors in a manner that recognizes, affirms and values the worth of each individual.
• Knowledge and skills to work with children, young adults and families.
• Valid driver's license, automobile, and insurance sufficient to meet agency requirements.
• Meet all the employee requirements including references, criminal background check, drug test, and driver’s license check.

• Maintain a caseload of approximately 8 children and families.
• Contact enrolled children and families within 48 hours of enrollment, and set up initial visit within 5 business days.
• Schedule and attend weekly visits with assigned children and families; facilitate monthly Child and Family Team Meetings.
• Assemble a Child and Family Team within two weeks of enrollment.
• Within 30 days, coordinate the meeting of the Child and Family Team and develop the initial Plan of Care including a 24-hour Crisis Plan.
• Monitor services and act as liaison for new services/resources.
• Advocate for Children and Families across home, school, court, and community settings.
• Attend all scheduled court hearings as a Wraparound Milwaukee representative.
• Seek community resources with Team assistance and modify SAR as needed.
• Provide or arrange transportation for youth to appointments and services; assist with transportation planning.
• Provide or secure crisis/emergency services via in-person, phone, or on-call systems.
• Complete all necessary paperwork per agency requirements (Plans of Care, SARs, Referrals, Progress Notes, etc.).
• Maintain accurate online family demographic and contact information.
• Collaborate with other necessary individuals the youth and family may contact.
• Keep Wraparound Milwaukee informed of emergencies or critical incidents.
