Waukesha County is seeking a Senior Care Manager (CST Care Coordinator) to provide family-centered, strengths-based case management for youths with complex behavioral health needs. Services span office, home, and community settings with potential nonstandard hours.

The role emphasizes collaboration across systems, adherence to CST wraparound values, and comprehensive documentation in the electronic health record.

Formación

• Bachelor’s degree in sociology, social work, psychology, nursing, criminal justice, or related field.
• Two years of responsible work experience in social work, counseling, nursing, criminal justice, or related field.
• Master’s degree may substitute for one year of work experience.
• Social work certification may substitute for one year of work experience.
• Valid driver’s license.

Responsabilidades

• Coordinate CST referrals, intake, screening, follow-up, and caseload management.
• Lead coordinated service teams with youth, families, natural supports, providers, schools, law enforcement, and community partners.
• Develop safety plans and crisis response plans; provide crisis intervention and referrals.
• Maintain enrollment data and complete local/state reports; support program monitoring and evaluation.
• Develop CST policies, procedures, interagency agreements, and training materials.
• Collaborate with families, schools, and partners; participate in case-planning and supervision meetings.
