Job summary

Canopy Children's Solutions is seeking a Wraparound Facilitator to engage families, develop a strength-based plan, and assemble a Wraparound Team to support children with complex mental health needs.

You will coordinate with family, youth, and community partners to create a holistic single care plan and conduct regular team meetings, ensuring crisis plans and documentation are completed promptly. A Bachelor’s in Social Work or related field is required.

Qualifications

• Bachelor’s degree in Social Work or related field is required.
• Experience working directly with children and families and formal/informal systems is required.
• Must complete the PCCSS trainings and certification exam within 90 days of hire.
• Flexible schedule to accommodate evenings and weekends.
• Must be at least 21 years of age, have a valid Mississippi driver license, good driving record, proof of auto liability insurance, and willingness to use personal vehicle for organization business.

Responsibilities

• Engage the family to begin the process of high-fidelity Wraparound and educate family, child/youth, and team members about the Wraparound process.
• Gather information for the family story and help develop their family vision for the first 30-day Wraparound Team Meeting.
• Complete the initial Crisis Management Plan with the family within 24 hours of admission.
• Engage families to identify supports and have them present at the initial Wraparound Team Meeting.
• Maintain team cohesiveness and foster positive communication between team and families.
• Facilitate a Wraparound Team Meeting at least every 30 days, especially in early development stages.

Education

Bachelor’s degree in Social Work or related field

Mississippi Department of Mental Health PCCSS trainings and certification exam
