Job summary

One Hope United seeks an Intensive Care Coordinator to deliver community-based Wraparound services for youth under the Pathways program, guided by CCSW supervision. The role emphasizes family-focused, youth-guided, strengths-based, trauma-informed care delivery in collaboration with multiple service providers.

You will maintain a caseload in Tier level Wraparound, conduct strength assessments, and coordinate plans of care and crisis prevention with families and teams.

Qualifications

• Bachelor’s degree in psychology, social work, counseling, education or similar.
• Two years of care-coordination experience, preferred, including family team meetings.
• Valid driver’s license with suitable auto insurance coverage.

Responsibilities

• Coordinate care with Wraparound/Pathways services following fidelity guidelines.
• Facilitate Child and Family Team Meetings and develop Plans of Care.
• Engage youth and families, assess strengths and target needs using IM CANs.
• Collaborate with providers to monitor progress and update plans.
• Advocate for youth in the least restrictive setting and crisis planning.
