Job summary

Patriot Enterprises is seeking Recovery Care Coordinators (RCCs) to advocate for wounded warriors and their families, coordinating services with DoD, VA, and NGOs. You will manage case data, provide monthly updates, and support complex benefits and medical processes.

Role requires strong communication, organization, and cross-agency collaboration, with frequent travel and a background check. Bachelor’s degree in behavioral or social science and 3 years in related field preferred.

Qualifications

• Experience in care coordination/case management.
• Excellent organization and communication skills.
• Interpersonal skills with ability to work with multiple agencies to accomplish objectives.
• MS Office proficiency and experience with Database Systems.
• Frequent travel is required.
• US Citizenship required; background check clearance.
• Bachelor's degree in behavioral or social science, preferred emphasis in case management or social work; 3 years experience.

Responsibilities

• Provides direct advocacy to support RSMs and their families through coordination of services and resources from governmental and non-governmental agencies.
• Inputs and updates all RSM case data in the case database and provides monthly status updates on active cases.
• Assists with health care, benefits, pay, entitlements, legal, and medical/physical evaluation board issues.
• Collaborates with DOD, VA, and NGOs to maximize benefits for the Wounded Warrior and families.
• Communicates with a multidisciplinary team throughout the continuum of care.
• Guides on high-visibility issues related to RSM.
• Ensures Comprehensive Recovery Plans (CRP) are developed/updated per regulations.
• Collects productivity and performance data.
