Job summary

Hartford HealthCare at Home seeks a Wound Program Specialist to lead prevention and management of skin breakdown in the home care setting. You will develop, coordinate, and implement multidisciplinary wound care plans to improve patient outcomes.

RN with WOCN certification is required; two years of nursing experience in clinical specialty area, home care preferred. You will drive quality improvement, educate staff, and collaborate with community partners to coordinate patient care.

Qualifications

• Bachelor’s Degree in nursing required.
• RN license; WOCN certification within 6 months of hire.
• Strong written and verbal communication skills.
• 2+ years nursing experience in clinical specialty area; home care preferred.

Responsibilities

• Develop and coordinate agency-wide wound care programs.
• Provide expert consultation and hands-on care for patients with wounds.
• Establish multidisciplinary management plans and coordinate care.
• Lead quality improvement initiatives and staff education.
