Job summary

Hartford HealthCare at Home is seeking a Wound Program Specialist to develop and coordinate agency‑wide wound prevention and management initiatives within a patient‑centered care model.

The role provides expert consultation and hands‑on care to acute and chronic wounds, coordinating multidisciplinary plans and education to improve outcomes. RN with WOCN certification is required, MSN preferred.

Qualifications

• Bachelor’s degree in nursing required; WOCN certification required; MSN preferred.
• Minimum two years nursing experience in a clinical specialty; two years in home care preferred.
• RN licensure; obtain Wound/Ostomy certification within 6 months of hire.
• Strong written and verbal communication skills.
• Knowledge of wound care standards and ability to balance quality, financial and patient outcomes.

Responsibilities

• Develops and coordinates agency wide wound prevention and management programs.
• Provides expert consultation and hands‑on care for patients with wounds.
• Establishes and coordinates multidisciplinary management plans.
• Develops educational programs for patients, staff and providers.
• Coordinates quality improvement initiatives to optimize wound outcomes.
• Advocates for patients with skin and wound care needs.
• Participates in performance improvement and staff training.
