Job summary

Hartford HealthCare at Home seeks a Wound Program Specialist to develop and coordinate agency‑wide programs for prevention and management of skin breakdown. You will provide expert consultation and hands‑on care, coordinate multidisciplinary management plans, and drive education for patients and staff across the region.

Qualified candidates hold a Bachelor’s in Nursing with RN licensure, WOCN certification, and at least two years of home health nursing experience.

Qualifications

• Bachelor’s Degree in nursing required.
• WOCN certification required.
• MSN preferred.

Responsibilities

• Reviews quality reporting data for deficiencies and barriers to safety.
• Develops an interdisciplinary approach to client care between Wound program, HHCAH staff and community partners.
