Job summary

Hartford HealthCare at Home is seeking a Wound Program Specialist with an RN background to lead comprehensive wound care initiatives across our agency. You will develop care plans, coordinate multidisciplinary teams, and promote best practices in wound prevention and management.

Requirements include a Bachelor’s degree in nursing, RN license, WOCN certification, and a minimum of two years in clinical nursing with homecare experience.

Qualifications

• Education: RN required with Bachelor’s Degree in nursing; WOCN certification required; MSN preferred.
• Experience: Minimum 2 years nursing in clinical specialty; minimum 2 years in homecare preferred.
• Licensure, Certification, Registration: RN; Wound/Ostomy certification within 6 months of hire required.
• Language Skills: Strong written and verbal communication skills.
• Knowledge, Skills and Ability: Ability to communicate across organization; leadership for patient outcomes; knowledge of wound care standards; MS Office proficiency.

Responsibilities

• Develops and coordinates interdisciplinary wound care programs and protocols.
• Provides expert consultation and hands-on wound care as needed.
• Coordinates care for patient populations and tracks regional wound program metrics.
• Supports education programs for patients, staff, and other providers.
• Participates in performance improvement and quality initiatives (H3W).
• Maintains collaboration with HHCAH clinical staff and community partners.
