Job summary

Hartford HealthCare at Home is seeking a Wound Program Specialist to develop and coordinate agency-wide programs for prevention and management of skin breakdown. You will provide expert consultation and hands-on care for acute and chronic wounds, and coordinate multidisciplinary care plans across the organization.

Responsibilities include leading quality improvement initiatives, developing educational programs, and collaborating with community partners to improve patient outcomes.

Qualifications

• RN required; active license in good standing.
• WOCN certification required or obtain within 6 months of hire.
• Minimum two years nursing experience in a clinical specialty.
• Two years in homecare preferred; strong communication skills.

Responsibilities

• Develops and coordinates agency-wide wound prevention and management programs.
• Provides expert consultation and hands-on wound care for patients.
• Coordinates multidisciplinary care plans and quality improvement initiatives.
• Develops educational programs for patients, staff and providers.
• Reviews regional wound program metrics to ensure quality and outcomes.
• Participates in training, coaching and onboarding for the wound program.
