Job summary

Hartford HealthCare at Home is seeking a Wound Program Specialist to develop and coordinate agency-wide wound prevention and management programs. You will provide expert consultation and hands-on care for patients with acute or chronic wounds, and lead interdisciplinary efforts to improve outcomes across the home care network.

The role requires an RN with WOCN certification, Bachelor’s in nursing, and at least two years in a clinical specialty (home care preferred).

Qualifications

• RN required with a Bachelor’s Degree in nursing.
• WOCN certification required; MSN preferred.
• Minimum two years nursing experience in clinical specialty area; home care experience preferred.

Responsibilities

• Develop and coordinate agency-wide wound prevention and management programs.
• Provide expert consultation and hands-on wound care for patients.
• Coordinate multi-disciplinary care plans and education for staff and care partners.
• Review regional wound program performance and participate in improvement initiatives.
