Job summary

Hartford HealthCare at Home seeks a Wound Program Specialist to lead and coordinate agency‑wide wound care initiatives. You will develop prevention and management protocols, provide expert consultation, and work with multidisciplinary teams to optimize patient outcomes in home care settings.

The role requires RN licensure, WOCN certification, and 2+ years in clinical nursing, with a preference for home health experience.

Qualifications

• Bachelor’s Degree in Nursing and RN licensure.
• WOCN certification required; MSN preferred.
• Minimum of two years nursing experience in a clinical specialty area and homecare preferred.
• Strong written and verbal communication skills.

Responsibilities

• Develop and coordinate agency‑wide wound care programs and guidelines.
• Provide expert consultation and hands‑on care for patients with wounds.
• Coordinate multidisciplinary care plans and quality improvement initiatives.
• Advise physicians and wound clinics on treatment changes and protocol updates.
• Support staff development and competency in wound care practices.
