Job summary

Hartford HealthCare at Home seeks a Wound Program Specialist to develop and coordinate agency‑wide wound care programs. You will provide expert consultation and hands‑on care, coordinating multi‑disciplinary plans and standard protocols to improve skin and wound outcomes.

The role emphasizes education for patients and staff, quality improvement efforts, and collaboration with community partners to optimize care delivery. RN with WOCN and two years in homecare are required.

Qualifications

Bachelor’s degree in nursing is required.
WOCN certification is required.
RN licensure is required; Wound/Ostomy certification within 6 months of hire.
Minimum 2 years nursing experience in clinical specialty; 2 years in homecare preferred.
MS Office proficiency (Outlook/Word/Excel/PowerPoint) expected.

Responsibilities

Develops and coordinates agency‑wide programs for prevention and management of skin breakdown.
Provides expert consultation and/or hands‑on wound care for patients.
Coordinates multi‑disciplinary care plans and quality improvement initiatives.
Develops educational programs for patients, staff and providers.
Acts as a consultant to coordinate client care with community partners.
