Job summary

Hartford HealthCare at Home is seeking a Wound Program Specialist to develop and coordinate agency-wide wound prevention and management initiatives in Connecticut.

The role provides expert consultation and hands-on care for patients with acute or chronic wounds, coordinating multi-disciplinary care plans and education for staff and providers. Strong nursing background with WOCN certification is required, with MSN preferred.

Qualifications

• Bachelor’s Degree in Nursing required; WOCN certification required; MSN preferred.
• RN license required; Wound/Ostomy certification within 6 months of hire.
• Minimum of two years nursing experience in a clinical specialty; two years in home care preferred.
• Strong written and verbal communication skills.

Responsibilities

• Develops and coordinates agency-wide wound prevention and management programs.
• Provides expert consultation and hands-on care for patients with wounds.
• Coordinates multi-disciplinary care plans and population health initiatives.
• Develops educational programs for patients, staff and providers.
• Tracks regional wound care program metrics and supports quality improvement.
• Coordinates training, onboarding and competency development for staff.
• Advocates for patients with skin and wound care needs and coordinates community partnerships.
