Job summary

Hartford HealthCare at Home is seeking a Wound Program Specialist to develop and coordinate agency-wide wound care programs. You will provide expert consultation and hands-on care for patients with acute or chronic wounds and coordinate multidisciplinary plans.

The role includes leading quality improvement initiatives, educating staff and patients, and collaborating with community partners to improve outcomes. RN with WOCN certification is required; MSN preferred.

Qualifications

Bachelor’s degree in nursing required; WOCN certification required; MSN preferred.
RN license required; wound/ostomy certification within 6 months of hire.
Strong written and verbal communication skills.
Ability to balance financial, quality, people and customer expectations.

Responsibilities

Develops and coordinates agency-wide programs for prevention and management of skin breakdown.
Provides expert consultation and hands-on care for patients with wounds.
Coordinates multidisciplinary, comprehensive management plans.
Develops educational programs for patients, staff and providers.
Leads quality improvement initiatives to optimize wound outcomes.
Acts as consultant to coordinate client care with community partners.
