Job summary

Healogics, Inc. seeks a Licensed Practical Nurse (LPN/LVN) for a part-time role at our Fort Worth wound care center. Under RN guidance, you will deliver wound care, assist with the vein clinic, and participate in intake, discharge, and case management as needed.

Candidates must be a graduate of an accredited LPN/LVN program with an active license in Texas and at least 2 years of LPN/LVN experience, preferably in SNF/LTACH settings. Wound care experience is a plus.

Qualifications

• High School Diploma or GED.
• Graduate from an accredited practical nursing program.
• Current LPN/LVN license in the state of practice.
• Two or more years’ LPN/LVN experience, including SNF/LTACH settings.
• Wound care experience preferred.

Responsibilities

• Under RN/Provider direction, provide wound-related patient care per BLS and hospital protocols.
• Assist in vein clinic as trained.
• May assist with patient intake and discharge.
• May assist with case management under RN direction per state regulations.
• Gathers patient information and required equipment.
• Reports quality of care issues to Clinical Nurse Manager.
• Communicate with team regarding interventions to ensure quality care.
• Perform hyperbaric oxygen therapy duties after training.
• May serve as Documentation Assistant (scribe) per policy.
• Perform other duties as assigned.
