Job summary

Healogics is seeking a licensed practical nurse (LPN) to join the Wound Care Center team in Richardson, TX. Under the direction of an RN or Provider, you will deliver wound-related care, assist with vein clinic duties, and support intake/discharge activities.

The role requires current LPN/LVN licensure and prior wound care or SNF/LTACH experience is preferred. Excellent communication and teamwork are essential as you coordinate with care teams and document care activities.

Qualifications

• Willingness to follow hospital and CMS guidelines.
• Competency in BLS and nursing procedures.
• Experience in SNF or LTACH settings is preferred.

Responsibilities

• Provide wound-related patient care under RN/Provider supervision.
• Assist in vein clinic after specialized training.
• Assist with intake and discharge processes.
• Participate in case management under RN guidance.
• Gather patient information and necessary equipment.
• Report quality of care issues to Nurse Manager.
• Performs hyperbaric therapy duties after training.
• May act as Documentation Assistant (scribe).
