Job summary

Healogics LLC is seeking a Wound Care Center LPN/LVN to provide wound-related nursing care under RN direction and hospital policy. You will assist in vein clinics, manage intake/discharge, and support case management following state regulations.

The role requires an LPN/LVN license, 2+ years of experience (SNF/LTACH preferred) and competency in BLS. You will operate in a patient care environment with training in hyperbaric therapy as needed.

Qualifications

• High school diploma or GED required.
• Graduate from an accredited practical nursing program.
• Current LPN/LVN license in state of practice.
• Minimum 2 years LPN/LVN experience; SNF/LTACH preferred.

Responsibilities

• Provide wound-related patient care under RN/provider and hospital protocols.
• Assist in vein clinic and intake/discharge per training.
• Support case management under RN direction.
• Gather patient information and supplies.
• Report quality of care issues to Clinical Nurse Manager.
• Communicate with team on therapies to ensure quality care.
• Perform hyperbaric therapy duties after training.
• Potentially act as Documentation Assistant (scribe).
