Job summary

Healogics, Inc. is seeking an experienced Licensed Practical Nurse (LPN) to join our wound care center. You will work under the direction of an RN or Provider to deliver wound care and related nursing services to patients in a fast-paced outpatient setting.

Responsibilities include collaborating with the care team, assisting with intake and discharge, and complying with BLS and hospital policies. Prior SNF/LTACH experience and wound care skills are preferred.

Qualifications

• High School Diploma or GED required.
• Graduate from an accredited practical nursing program/state approved program.
• Current LPN/LVN license in the state of practice.
• Two or more years’ LPN/LVN experience, including SNF/LTACH preferred.
• Wound care experience preferred.

Responsibilities

• Under supervision of RN/Provider, provide wound care per BLS and hospital policies.
• Assist in vein clinic after training.
• Assist with intake and discharge as assigned.
• Assist case management under RN direction per state regulations.
• Collect patient information and prepare needed equipment.
• Report quality issues to Clinical Nurse Manager.
• Communicate with the team about interventions to ensure quality care.
• Perform hyperbaric therapy duties as trained; may assume hyperbaric safety duties.
• May function as a scribe in accordance with policy.
• Other duties as delegated.
