Job summary

Healogics is seeking a Wound Care Center LPN to provide wound-related patient care under the direction of an RN or provider. The role supports vein clinic activities after required training and may assist with intake, discharge and case management according to policy.

Required are a high school diploma or GED, an accredited LPN program degree, and a current LPN/LVN license. At least 2 years’ LPN/LVN experience, with SNF/LTACH exposure, is preferred.

Qualifications

• High School Diploma or GED.
• Graduate from an accredited practical nursing program/state approved educational program.
• Current LPN/LVN license in the state of practice.
• Two or more years’ LPN/LVN experience, including SNF or LTACH setting.
• Wound care experience preferred.

Responsibilities

• Provide wound-related patient care under RN/provider supervision.
• Assist in vein clinic after Vein Clinic training.
• May assist with intake and discharge.
• May assist with case management under RN guidance.
• Gather patient information and necessary equipment.
• Report quality of care issues to Clinical Nurse Manager.
• Communicate with team about therapeutic interventions.
