Job summary

Healogics Wound Care & Hyperbaric Services LLC seeks an experienced LPN/LVN to support wound care patients in an outpatient setting. You will work under the RN or provider, assist in vein clinics, and help with intake, discharge, and case management while following hospital protocols.

The role requires an active LPN/LVN license, 2+ years of nursing experience (prefer SNF/LTACH), and BLS competency. Hyperbaric training will be provided; strong teamwork and communication are essential.

Qualifications

• High School Diploma or GED.
• Graduate from an accredited practical nursing program.
• Current LPN/LVN license in the state of practice and active throughout employment.
• Two or more years’ nursing experience, including SNF or LTACH settings.
• Wound care experience preferred.

Responsibilities

• Provide wound-care nursing under RN/provider direction per protocol.
• Assist in vein clinic as trained.
• Support intake and discharge processes as needed.
• Assist with case management under RN guidance and state rules.
• Gather patient information and prepare equipment for care.
• Document and report quality of care issues to supervisor.
• Perform hyperbaric therapy-related duties after training.
