Job summary

Healogics, LLC. is seeking an LPN/LVN to join a wound care-focused team at a site within New Jersey. You will provide wound-related patient care under the supervision of an RN or provider, assist in the vein clinic, and support intake, discharge, and documentation processes.

Candidates should have an active LPN/LVN license in NJ, 2+ years of LPN/LVN experience (SNF/LTACH preferred), and BLS certification. Wound care exposure is preferred. Training will cover hyperbaric tasks as needed.

Qualifications

• High School Diploma or GED completed.
• Graduate from an accredited practical nursing program/state approved educational program.
• Current LPN/LVN license in the state of practice, maintained throughout employment.
• Two or more years’ LPN/LVN experience, with SNF/LTACH exposure preferred.
• Wound care experience is preferred.

Responsibilities

• Provide wound-related patient care under RN/provider direction per BLS and policies.
• Assist in vein clinic after Vein Clinic training.
• Assist with intake and discharge as needed.
• Support case management under RN guidance per state rules.
• Greet and collect patient information.
• Gather equipment and supplies for RN or provider.
• Report quality of care issues to Clinical Nurse Manager.
• Communicate with team about interventions to ensure quality care.
• Perform hyperbaric-related duties after training, as needed.
• Possibly serve as Documentation Assistant (scribe) per policy.
• Perform other delegated duties.
