Job summary

Healogics is the largest provider of advanced wound care services in the United States, seeking an LPN/LVN to support wound care centers under RN supervision. The role involves direct patient care, intake/discharge tasks, and collaboration with the nursing team while adhering to hospital policies and CMS guidelines.

Responsibilities include wound care delivery, vein clinic support, documentation, and potential hyperbaric therapy-related duties after required training.

Qualifications

• High School Diploma or GED.
• Graduate from an accredited practical nursing program/state approved program.
• Current LPN or LVN license in the state of practice.
• Two (2) or more years' experience as LPN/LVN, including at least one year in SNF or LTACH setting.
• Wound care experience preferred.

Responsibilities

• Under the direction of the RN/Provider, provides wound-related patient care according to BLS and hospital protocols.
• Assist in vein clinic as assigned after Vein Clinic training completed.
• May assist with intake and discharge.
• May provide assistance with case management under RN guidance.
• Gathers patient information.
• Gathers necessary equipment and supplies for the RN or provider.
• Reports quality of care issues to Clinical Nurse Manager.
• Communicates with team members regarding therapeutic interventions to ensure quality of care.
• Performs hyperbaric oxygen therapy related duties after hyperbaric training.
• May perform hyperbaric safety director duties after training.
• May function as a Documentation Assistant (scribe).
• Performs other duties as delegated.
