Job summary

Healogics LLC is seeking an LPN/LVN to provide wound-related nursing care under an RN or provider. You will assist in the vein clinic, handle intake/discharge, and support case management in a fast-paced outpatient center.

Requirements include an active LPN/LVN license, 2+ years of LPN/LVN experience (SNF/LTACH preferred), and BLS. Prior wound care experience is preferred; duties align with hospital policies and CMS guidelines.

Qualifications

• Holds a current LPN/LVN license in the state of practice.
• 2+ years of LPN/LVN experience, with SNF/LTACH exposure preferred.
• Wound care experience is preferred.

Responsibilities

• Provide wound-related patient care under RN/provider guidance.
• Assist in vein clinic after training, as assigned.
• May assist with intake and discharge processes.
• Support case management under RN direction per state rules.
• Gather patient information and necessary equipment.
• Report care quality issues to the nurse manager.
• Maintain clear communication with the team on therapies and care.
