Job summary

Healogics is seeking an LPN to support wound care services under RN/Provider supervision in an outpatient center. Responsibilities include wound-related patient care, vein clinic assistance after training, intake/discharge support, and case management under RN guidance.

The role requires an LPN license in the state of practice, 2+ years of LPN/LVN experience (wound care preferred), and proficiency with BLS, organization, and multi-tasking in a fast-paced setting.

Qualifications

• High School Diploma or GED required.
• Graduate of an accredited practical nursing program/state approved educational program.
• Current LPN or LVN license in the state of practice.
• Two or more years’ experience as LPN/LVN, including one+ year in SNF or LTACH setting.
• Wound care experience preferred.

Responsibilities

• Under RN/Provider direction, provides wound-related patient care per BLS and hospital protocols.
• Assist in vein clinic after training; may assist with intake and discharge.
• May provide assistance with case management under RN supervision.
• Gathers patient information and necessary equipment/supplies.
• Reports quality of care issues to Clinical Nurse Manager.
• Communicates with team regarding therapeutic interventions.
• Performs hyperbaric oxygen therapy duties after hyperbaric training.
• May perform hyperbaric safety director duties after training.
• May function as a Documentation Assistant (scribe) per policy.
• Performs other duties as delegated.
