Job summary

Healogics is seeking an experienced LPN/LVN to join our wound care team in a dynamic outpatient setting. Under supervision of the RN/Provider, you will deliver wound-related nursing care and support vein clinics, intake, discharge, and case management as needed.

Qualifications

• Current Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license in the state of practice.
• Two or more years’ experience as LPN/LVN, including at least 1 year in SNF or LTACH settings.
• Wound care experience preferred.

Responsibilities

• Provide wound-related patient care under the direction of RN/Provider.
• Assist in vein clinic after Vein Clinic training completion.
• Assist with intake and discharge and case management as directed by RN.
• Gather patient information and necessary equipment for procedures.
• Report quality of care issues to Clinical Nurse Manager and communicate with team.
• Assist with hyperbaric therapy duties after training, and may perform safety director duties when trained.
• Possibly serve as a Documentation Assistant (scribe) per policy.
• Perform other duties as delegated.
