Job summary

Southern Maryland Tri-County Community Action Committee Inc in Hughesville, MD seeks a Workforce Case Manager to ensure timely, appropriate supportive services for Road to Careers participants and oversee transitions from training to employment.

The role requires conducting intake, coordinating literacy assessments, developing individualized service plans, coordinating transportation and other supports, and communicating with employers to support onboarding and post-placement success.

Qualifications

• High School Diploma or equivalent required.
• Two years' experience in case management or related field.
• Working knowledge of public assistance programs (TCA, SNAP, LIHEAP) and benefit transition considerations.
• ELDT instructor certification required within 90 days of hire.
• CPR and First Aid certifications required.
• Valid Maryland Driver's License required.

Responsibilities

• Provide participant-centered, barrier-responsive case management and initiate screening into opportunities.
• Conduct intake interviews and barrier assessments for all participants.
• Coordinate literacy/numeracy assessments with instructors and partner agencies.
• Develop an individualized supportive services plan for each participant.
• Coordinate transportation, DOT scheduling, employment readiness, and basic needs supports.
• Assist with benefit transitions to mitigate cliff effects and ensure continuity of services.
• Maintain documentation of services and referrals per grant reporting.
