Job summary

AOE International is seeking a Case Manager to carry a caseload from enrollment through at least one year post-completion. The role focuses on creating Individual Employment Plans, tracking progress through training and into work, and administering Barrier Reduction services to remove obstacles to success.

Qualifications

• Enables development and tracking of Individual Employment Plans
• Knows how to administer Barrier Reduction services
• Maintains accurate records and confidentiality

Responsibilities

• Complete needs assessments at enrollment and update throughout the program
• Develop Individual Employment Plan within 10 business days and amend at each transition
• Meet participants weekly during training and monitor progress
• Recommend Barrier Reduction services and document actions
• Coordinate referrals with partner providers
• Support transition services, job readiness, placement, and retention
• Conduct monthly follow-up for at least one year after completion
• Record services and outcomes in CEJA Reporting System within five business days
