Job summary

Wraparound Maryland, Inc. seeks a full-time Case Manager to support adults and families in Worcester County. You will provide intensive case management, connect clients with housing, employment, and services, and develop ISP plans with bi-weekly progress reviews.

Qualifications

• BA or BS degree in social work or related field; extensive experience in human services preferred but not required.

Responsibilities

• Provide intensive case management using a family-centered approach.
• Assist clients in obtaining goals leading to stability, housing, employment, and self-sufficiency.
• Link clients to health, mental health, education, employment, housing, and benefits; monitor attendance at life skills, parenting and employment readiness classes.
• Maintain accurate case records and progress notes; develop detailed ISP with goals and action plans to obtain permanent housing and self-sufficiency.
