Job summary

Ready 4 Work is seeking a dedicated Case Manager for our Maryland Day Program. You will work directly with clients to support recovery goals, including vocational, housing, and educational challenges, while promoting independence.

Responsibilities include assessments, tenancy support, care coordination, and collaboration with the care team. A Bachelor’s degree in a relevant field is required, with English communication and a valid driver’s license essential for daily operations.

Qualifications

• Bachelor’s degree in psychology, counseling, social work, rehabilitation, or behavioral science.
• Experience working with individuals with serious mental illness or in community support settings preferred.
• Must be at least 21 years old with a valid driver’s license and clean driving record.
• Ability to pass a criminal background screening and communicate effectively in English.

Responsibilities

• Perform assessments, interviews, and documentation for clients.
• Provide tenancy support and assist with housing stability.
• Help clients access medical care, social services, and housing resources.
• Facilitate rehabilitation groups and individual meetings.
• Document interactions and develop person-centered plans with measurable goals.
• Collaborate with therapists, psychiatrists, LPNs, and family members.
• Support daily living activities and crisis intervention.
• Coordinate transportation to appointments and services.
• Monitor medications and maintain updated lists.
